TERMS AND CONDITIONS

Definition of Repair:
  • United RF, LLC. will repair all items to meet functional specifications.
  • All units are tested using the same performance standards as new units.
  • Upgrades are only completed at customer request or when safety and reliability issues exist. When possible, upgrades are recommended to improve performance.
  • Units are cleaned as required, but standard repairs do not include aesthetic refurbishment (i.e. repainting, panel replacement, etc.) unless requested by the customer.
RMA Process:
  • All repairs must be assigned a Return Material Authorization (RMA) number before they are shipped to our repair facility.
  • To request a RMA number, please contact United RF, LLC. at (719) 393-7373 or email support@unitedrf.com
  • The following information is required for us to provide a RMA number:
    – Billing Address.
    – Shipping Address.
    – Contact Person.
  • Please provide the contact’s name, phone, fax and e-mail. This person will be
    contacted for repair coordination and approval of repair charges.
  • Part Number of unit to be returned.
  • Serial Number of unit to be returned.
  • Reason unit being returned. Please provide a detailed description of the failure.
  • To expedite the repair process it is strongly suggested a Not To Exceed (NTE) Purchase Order (PO) be provided for every repair unit.
Returning a Unit for Repair:
  • Please be sure to use proper packaging.
  • Customer is responsible to pay for all repairs resulting from shipping damage to the unit.
  • Customer is responsible for all shipping charges to return a unit. Ship unit as desired.

  • Please be sure to reference the RMA number on the box and shipping paperwork.
Health & Safety Form:
  • A Health & Safety Form must be completed for every unit returned for repair.
  • The Health & Safety form should be included with the unit when returned or provided to the Service
    Administrator prior to return.
  • Service Administrators will provide Health & Safety Forms upon request.
Repair Charges:
  • United RF, LLC. accepted payment terms are due upon receipt of invoice.
  • Repair Charges are calculated based on the price of the repair components and our standard labor rate of $95.00 per hour or a flat fee rate.
  • An Evaluation Fee exists for every Non-Warranty repair unit.
  • The Evaluation Fee is $150.00 for Microwave, Ozone, Astron and all other Semiconductor units.
  • The Evaluation Fee is $500 for all Medical MRI units.
  • The Evaluation Fee will be charged in cases of No Problem Found (NPF), minimal repairs, scrap units, and units returned without repair.
  • When the repair charge exceeds the evaluation fee, the evaluation fee is not charged.
  • The Evaluation Fee will be charged for NPF units regardless of Warranty status.
  • For Non-Warranty repairs a Service Cost Authorization (SCA) is sent via fax or e-mail after the evaluation of the unit is completed. The SCA contains problem found, repair required and the expected repair charge.
  • After the SCA is sent, the repair is placed on hold until the SCA is marked with the desired disposition for the repair (i.e. approval to repair, request to scrap, etc.), signed and returned.
  • No repairs will be completed without receipt of a PO valid for the repair charges.
  • United RF, LLC. can only accept PO s. Sorry, but Credit Cards cannot be accepted.
  • In cases where a Not To Exceed (NTE) PO has been received for the repair, we will proceed immediately if the repair charges are below the NTE amount. SCA is still sent for informational purposes only.
  • If the repair charges exceed the NTE amount, the SCA will be sent and the repair placed on hold per the normal approval process.
  • SCA’s should be returned within 2 weeks of receipt.
  • All Warranty repair units will be completed as quickly as possible. SCA’s are not sent for warranty repairs, but can be provided upon request.
  • Revised SCA s may have to be sent and approved if additional problems are found during the repair process resulting in additional repair charges. We attempt to make every SCA accurate when first sent, but additional problems are sometimes unavoidable and are often undetectable during the evaluation process.
  • A Fixed Repair Charge is only applied on all products United RF, LLC. repairs.
Repair Terms and Additional Fees:
  • Standard Repair Turnaround Time is 7 business days.
  • Urgent Repair Turnaround Time is 3 business days.
  • Urgent repairs are charged an Expedite Fee of $500.00. The Expedite Fee is only charged when the repair is completed within 3 business days (excluding time the repair unit was on hold awaiting approval of repair charges).
  • Failure Analysis Reports (FAR) are provided upon request at no charge for Warranty units.
  • FAR’s are provided for Non-Warranty units for a fee of $500.00 per unit. Please note, FAR’s will lengthen the repair turnaround time.
  • Units received that have been used in a Semiconductor Copper Process must be specially handled to avoid cross contamination. Non-Warranty, Copper contaminated units will be charged a Copper Handling Fee of $250.00 per unit.
  • Repair Reports containing problem found, repair completed and materials used are provided upon request.
  • The customer is responsible for all shipping charges to return Non-Warranty repairs. Units will be shipped per customer’s instructions.
  • All units are shipped FOB Fountain, CO.
  • All failed components removed from a repair unit are scrapped at United RF, LLC. and cannot be returned with the repaired unit.
Repair Warranty:
  • All repair units receive a 180-Day Repair Warranty unless otherwise specified by United RF, LLC..
  • The Repair Warranty begins on the date the unit is shipped back to the customer.
  • The Repair Warranty only covers components that were repaired on the unit. Failure of different components on a unit is not covered by the Repair Warranty.
  • Consumable items are not covered by the Repair Warranty.
